We also ask that you provide us with an up-to-date medication list at the first appointment. It is important that you continue to keep us informed about any medication changes or any changes to your health at subsequent visits. We will ask you to update your information during each dental visit.
Change of Appointments
At Comox Valley Dental Centre, we schedule appointments for you. When you schedule your appointment, you will receive an appointment card with the details of your upcoming visit.
As a courtesy reminder, you will be contacted by phone, email or text, a few days prior to your upcoming appointment to confirm the time and date.
We would appreciate a minimum of 2 full business days’ notice to change or cancel each appointment.
As a courtesy to our patients, we will assist you in filing your insurance forms related to the dental care you received at our office. Many plans may not cover the entire cost of the specific treatment that you require.
It is your responsibility to know the details of your benefit plan. We do not have access to this information.
Payment for the portion of cost not covered by your benefit plan is expected on the day of treatment, unless other financial arrangements have been made with us in advance.
